Anchorpoint Support Coordination is an NDIS registered provider delivering both Support Coordination (Level 2) and Specialist Support Coordination (Level 3).

We are a team of experienced, independent Support Coordinators providing tailored support to help participants understand their plans, connect with the right services, and navigate the NDIS with confidence.

We support people across a range of needs — from implementing everyday supports through to managing complex situations requiring intensive coordination and risk management.

Support Coordination (Level 2)

Support Coordination helps participants understand and implement their NDIS plans, so they can access the right supports and get the best outcomes from their funding.

We assist with:

  • Connecting with appropriate supports and services: identifying and linking participants to providers, allied health professionals, community activities and local services that match goals and needs.

  • Coordinating service delivery: organising appointments, clarifying roles between providers, monitoring supports and helping resolve issues to ensure consistent, effective service delivery.

  • Building confidence to manage supports: developing skills and knowledge so participants can make informed choices, self‑manage aspects of their plans where desired, and gradually take greater control.

  • Strengthening informal and community supports: mapping and enhancing family, friends and community networks to provide sustainable day‑to‑day support alongside formal services.

  • Help interpret plan budgets and funding categories to maximise value.

  • Liaise with multiple providers and stakeholders to reduce duplication and gaps.
